Job summary

Carle Health is seeking an experienced Home Care RN/Field Case Manager to join our interdisciplinary home care team. You will coordinate patient care plans, educate patients and families, and perform skilled nursing assessments across our service area.

You will manage wound care, PICC/Hickman care, IV infusions, and OASIS/485 documentation while collaborating with LPNs, HHAs, PT/OT, MSW, SLP and physicians to ensure seamless transitions and high-quality outcomes.

Qualifications

IL RN licensure; BLS within 30 days
Driver's license and proof of auto insurance
College Diploma in Nursing; 1 year nursing experience

Responsibilities

Coordinate and provide direct and indirect patient care using the nursing process.
Perform complex wound care including wound vac, dressings, and catheters.
Monitor and manage central lines (PICC/Hickman) and IV infusions.
Complete OASIS/485 changes within 24 hours.
Communicate with MD, care coordinators, and families; document admissions.
Supervise Home Health Aide and oversee care plans.
Ensure regulatory compliance and accurate documentation.
Assign Home Health Aide tasks and provide supervision in patients' homes.
