Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to perform initial patient assessments, SOC/OASIS, and follow-up visits in the Gilroy and Morgan Hill area. You will coordinate care with physicians and the clinical team, manage IV infusions when needed, and document patient progress.

The role requires an CA RN license, BLS/CPR, and a driver’s license. Flexible scheduling and mileage reimbursement are part of the package, with opportunities for

Qualifications

• CA RN license required and current.
• BLS/CPR certification required.
• Experience in home health or case management preferred.
• Strong communication and collaboration with interdisciplinary teams.

Responsibilities

• Conduct Start of Care (SOC) admissions and OASIS assessments.
• Perform follow-ups and evaluation visits in the territory.
• Make home visits to patients in Gilroy/Morgan Hill area.
• Reconcile medications, wound care, and treatments as assigned.
• Monitor patient conditions and report changes to the Clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Initiate IV therapy safely when required and per protocol.
• Document observations and interventions accurately.
