Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager for the Berkeley area. The role requires performing SOC assessments, home visits, IV infusions, and coordination with the clinical team to deliver high-quality home health care.

Ideal candidates hold a CA RN license, BLS/CPR certification, and a CA Driver's License, with experience in OASIS, high-acuity patients, and interdisciplinary collaboration.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting Start of Care (SOC) - OASIS is necessary.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Previous experience with IV Medication infusions highly preferred.
• Proficiencies initiating intravenous (IV) therapy is a plus.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Follow-up and evaluation visits in designated territory as needed.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patients’ conditions and report changes to the Clinical team.
• Collaborate with physicians and agency staff to deliver quality care.
• Initiate and administer IV medications safely per protocols.
• Document observations, interventions, and evaluations.
