Job summary

21st Century Home Health Services (21HHS) is seeking an experienced Registered Nurse to join our field-based care team in the San Francisco Bay Area. You will perform initial patient assessments, develop advanced care plans, and collaborate with an interdisciplinary team to improve outcomes.

Requirements include CA RN license, BLS/CPR, and a minimum of one year of home health or case management experience. We offer comprehensive benefits, flexible scheduling, and growth opportunities.

Qualifications

• Minimum of one year of Home Health Case Management experience preferred.
• CA RN license in good standing with active CPR/BLS certification.
• Experience supervising or mentoring nurses is highly preferred.

Responsibilities

• Perform a full assessment of the patient and family during initial visit.
• Develop an advanced plan of care including rehabilitation.
• Work with patients and family to achieve improvement and/or stability.
• Case manage the patient care episode with interdisciplinary clinical team.
• Prepare clinical notes, orders and charting in a compliant time frame.
• Collaborate with physicians and agency staff to deliver the best care.
