Job summary

21st Century Home Health Services (21HHS) seeks a Part-time Registered Nurse Case Manager to join our field team serving Gilroy/Morgan Hill. You will perform SOC admissions, in-home visits, and IV infusions while coordinating with physicians and the care team to deliver high-quality patient care.

Current CA RN license with BLS/CPR is required. This role offers mileage reimbursement, a company iPad, and strong clinical leadership opportunities within a supportive environment.

Qualifications

• Active CA RN license with BLS/CPR certification.
• Experience in home health or case management is preferred.
• Ability to travel to patient homes within territory.

Responsibilities

• Perform Start of Care (SOC) Admissions and OASIS assessments.
• Conduct follow-up visits and in-home evaluations.
• Visit patients within a 5-15 mile radius; schedule flexibility.
• Coordinate with physicians and the care team to deliver quality care.
• Administer IV medications per protocol.
• Document observations and interventions accurately.
• Mileage reimbursement provided.
