The ideal candidate holds an Associate's in Nursing with RN licensure, with 2+ years in hospital care management or utilization management and strong communication skills for diverse patient needs.

Qualifications

• RN license required with strong discharge planning knowledge.
• Experience in hospital care management or utilization management preferred.
• Ability to analyze complex patient needs and coordinate post-acute care.
• Excellent communication with patients, families, and multidisciplinary teams.
• Proficiency in electronic medical records and office tools.

Responsibilities

• Participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness, resources, and care options.
• Assesses readmissions and identifies reasons with the care team.
• Facilitates patient and family care conferences with the team.
• Documents discharge planning evaluations and barriers to care progression.
• Coordinates post-acute care arrangements and ensures timely transitions.
