AdventHealth in Ocala, FL seeks an experienced Care Management RN to ensure safe discharge planning and seamless care transitions. You will participate in rounds, educate patients and families, and coordinate resources to address post-acute needs.

The role requires 2+ years of hospital nursing experience, RN licensure, and strong communication across diverse backgrounds. A focus on care coordination and patient advocacy is essential.

Formación

• 2+ medical/hospital nursing experience.
• Prior Care Management/Utilization Management RN experience preferred.
• RN licensed and BLS/ CCM preferred.

Responsabilidades

• Participates in multidisciplinary rounds to review changes in patient status and discharge plans for timely transition.
• Educates patients and families on emotional, social, and financial impacts of illness and mobilizes resources.
• Assesses readmitted patients for reasons and coordinates care transition.
• Organizes and facilitates discharge planning conferences with the team.
• Documents discharge plans and barriers to progression of care in standard records.
• Communicates post-acute care needs to payors for authorization as needed.
• Reviews records to integrate clinical, social, and financial factors into the discharge plan.
• Develops discharge plans with contingency plans for timely care coordination.
