Job summary

AdventHealth in Ocala, FL is seeking an experienced RN Care Manager to coordinate discharge planning and post-acute care for medically complex patients. You will participate in rounds, educate families, and ensure timely transitions while coordinating with payors and community resources.

Requires an active RN license, 2+ years hospital nursing, and prior care management experience. This 52-week weekend contract offers day shifts and part-time hours on weekends.

Qualifications

• Associates of Nursing required.
• Bachelor’s in Nursing preferred.
• Active RN license required.
• 2+ years of medical/hospital nursing experience required.
• Prior hospital Care Management/Utilization Management experience required.

Responsibilities

• Participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families about illness impacts and mobilizes resources.
• Assesses readmission factors for patients and families.
• Organizes and facilitates patient/family care conferences with the team.
• Documents discharge planning evaluations and barrier tracking.
• Coordinates post-acute care authorizations with Payors as needed.
• Develops discharge plans with contingency plans for hospital stay.
• Ensures continuity of care through communication with post-acute services.
