The ideal candidate has 2+ years of RN experience in a hospital setting, strong leadership, problem-solving, and communication skills.

Qualifications

• Registered Nurse with RN license.
• 2+ years of medical/hospital RN experience.
• Care management/utilization management experience preferred.

Responsibilities

• Participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families about illness impacts and resources.
• Documents discharge planning evaluation and barriers to progression.
• Engages with community resources for post-acute care.
