Job summary
AdventHealth in Orlando, FL is seeking a Registered Nurse Care Manager to coordinate discharge planning and ensure safe transitions for patients across the continuum. The role requires strong leadership, critical thinking, and collaboration with multidisciplinary teams to optimize patient outcomes.
You will assess readmission risks, engage patients and families, and connect to post-acute services while navigating CMS requirements.
Qualifications
- Associates degree in nursing required or equivalent
- Bachelor’s degree in nursing preferred
- Current Registered Nurse licensure in Florida or eligible
- BLS certification preferred; CCM/ACM certifications preferred
Responsibilities
- Coordinate discharge planning and transitions for assigned patients
- Actively participate in multidisciplinary rounds to review patient status and resources
- Educate patients and families on illness, care options, and post-acute services
- Communicate with payors and post-acute care facilities to ensure continuity of care
- Document discharge planning evaluations, barriers, and progress in the medical record
