Job summary

Qualifications

• Registered Nurse (RN) required.
• 2+ years of medical/hospital nursing experience.
• Care management/Utilization management experience preferred.
• Florida Level 2 background screening (fingerprinting) required for certain positions.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status, progression and plan of care.
• Develops discharge plans and coordinates post-acute services to ensure timely care transitions.
• Communicates with patients, families, and Payors regarding post-acute care needs and authorization requirements.
• Leverages standard work to document discharge planning evaluations and progress notes.
