AdventHealth in Florida is seeking an experienced RN Case Manager to coordinate discharge planning and ensure timely transitions for patients across inpatient and observation units.

You will assess patient and family needs, educate on options, and collaborate with social work, nursing, and physicians to arrange post-acute care services. Strong communication and organizational skills are essential for success in a fast-paced hospital setting.

Qualifications

• RN licensed in Florida or eligible to obtain license
• Minimum 2 years of medical/hospital nursing experience
• Experience in Care Management/Utilization Management preferred
• Strong discharge planning and patient education skills

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status and discharge plans
• Educates patients and families on emotional, social, and financial aspects of illness
• Documents discharge planning evaluations and progress in medical records
• Communicates with payors for post-acute care authorizations when needed
• Develops discharge plans with contingency plans to ensure timely progression of care
