Job summary

AdventHealth in Orlando, FL is seeking a qualified RN to supervise discharge planning and social services across inpatient, observation, and ED units. You will lead care management strategies, participate in multidisciplinary rounds, and ensure timely, compliant documentation and escalation of issues.

The role requires RN licensure, 1+ year in care management, and preferred ACM/CCM credentials. On-site, full-time with day shifts and Florida background screening requirements.

Qualifications

• RN with active license and inpatient care experience.
• Bachelors preferred; associates required for RN licensure.
• ACM/CCM certifications preferred for advancement.
• Experience with discharge planning and care management processes.
• Ability to lead teams within regulatory frameworks.

Responsibilities

• Oversee discharge planning and social services in inpatient, observation, and ED.
• Lead care management rounds and collaborate with facility management.
• Ensure compliant documentation and participation in rounds.
• Facilitate complex transitions and care conferences as needed.
• Identify quality issues and escalate appropriately.
