Qualifications

• RN required for discharge planning and care coordination.
• 2+ medical/hospital nursing experience.
• Prior hospital care management/utilization management experience preferred.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families about illness impacts and coordinates post-acute resources.
• Assesses readmission causes and documents discharge plans and barriers to progression.
