Elite Hospice, part of LHC Group within the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends.

In this role you will triage patient needs by phone, perform nursing evaluations within 48 hours, and conduct afterhours visits, admissions, and recertifications as required. Documentation of care and responsive, respectful service are essential to support patients and families.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can visit in person.
• Respond to crisis calls from patients and families after hours with urgency.
• Make initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Perform afterhours visits including admission, death, recertification and routine visits.
