Job summary

UnitedHealth Group's Access Hospice Care, part of LHC Group, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and their families after regular hours, including nights and weekends.

Responsibilities include triaging needs by phone, crisis response, initial nursing evaluation within 48 hours, and conducting afterhours visits such as admissions, recertifications, and routine visits.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver’s license and reliable vehicle or public transportation.
• CPR certification preferred or can be completed within 90 days of hire.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to after-hours crisis calls from patients and families.
• Perform initial nursing evaluation within 48 hours of referral when assigned.
• Conduct after-hours visits including admissions, death visits, recertifications and routine visits.
• Identify patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions and responses to hospice care.
