Job summary

Optum is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care for patients and families after regular hours, including nights and weekends. The role requires timely nursing evaluations and ability to manage afterhours visits, with a focus on patient-centered care.

Responsibilities include triage over the phone, crisis response, and ensuring timely eligibility determinations. A valid RN license, driver’s license, and CPR certification are preferred, with wage details noted

Qualifications

• Current unrestricted RN licensure in the state of practice (required).
• Current driver’s license and reliable vehicle or public transportation (required).
• CPR certification preferred or to be completed within 90 days of hire.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls from patients and families after hours with urgency.
• Conduct initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Perform afterhours visits including admissions, death, recertification, and routine visits.
• Assess and reassess patient/family physical, psychosocial, emotional, and environmental needs at least every 15 days.
• Document problems, goals, interventions, and patient/family response to hospice care.
