Job summary

Elite Hospice, part of LHC Group and UnitedHealthcare family, seeks a Per Diem After-hours Registered Nurse to deliver comprehensive hospice care for patients and families after regular hours, including nights and weekends.

Responsibilities include tele-triage, urgent crisis response, initial evaluation within 48 hours, and after-hours visits with thorough documentation of patient needs and outcomes.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR Certification.
• Minimum One year of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond to crisis calls from patients and families after hours with urgency.
• Make the initial nursing evaluation within 48 hours of referral as part of a patient visit.
• Perform afterhours visits including admission, death, recertification, and routine visits.
• Identify patient/family needs and reassess every 15 days; document problems, goals, interventions, and responses.
