Job summary

Elite Hospice, part of LHC Group and a member of the UnitedHealth Group family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends. Responsibilities include phone triage, crisis response, and in-home visits with timely documentation.

Required is an unrestricted RN license in the state of practice and a valid driver's license with access to a vehicle. Preference for CPR or 1+ year clinical experience.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Responds to crisis calls from patients and families after hours with urgency.
• Conducts initial nursing evaluation within 48 hours of referral when assigned.
• Performs afterhours visits including admission, death, recertification, and routine visits.
• Identifies patient/family needs and reassesses every 15 days.
• Documents problems, goals, interventions, and patient/family response.
• Provide care in any home situation regardless of demographics.
