Job summary

Heart n Home Hospice, part of the LHC Group and Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families after regular hours, including nights and weekends. You will triage over the phone, assess needs, and conduct afterhours visits, with emphasis on timely evaluations within 48 hours of referral and caring for diverse home situations.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Triage patient needs over the phone until an in-person visit.
• Respond to crisis calls after hours with urgency.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours visits including admissions, death, recertification, and routine visits.
• Assess and document physical, psychosocial, emotional, and environmental needs of patients and families at least every 15 days.
• Document problems, goals, interventions, and patient/family response.
• Provide care in diverse home situations regardless of demographic factors.
