Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families after regular hours, including nights and weekends. You will triage needs by phone, perform afterhours visits, and document care plans and patient responses.

Responsibilities include evaluating eligibility for hospice services within 48 hours of referral, handling crisis calls, and ensuring care milestones are met while navigating diverse home environments.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver’s license, valid vehicle insurance, and access to reliable transportation.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse is available.
• Respond promptly and with urgency to afterhours crisis calls from patients and families.
• Conduct initial nursing evaluation within 48 hours of referral when assigned.
• Perform afterhours visits including admissions, death, recertification, and routine visits.
• Assess patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions, and responses to care.
