Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care for patients and families during nights and weekends. The role includes crisis triage by phone and conducting home visits to assess needs.

You will perform initial nursing evaluations within 48 hours of referral, handle afterhours admissions and recertifications, and document care plans while collaborating with families to address physical, psychosocial, and environmental needs.

Qualifications

• Current, unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or reliable public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can visit.
• Respond to afterhours crisis calls with urgency.
• Perform initial nursing evaluation within 48 hours of referral during patient visit.
• Conduct afterhours admissions, death, recertification, and routine visits.
• Assess and reassess physical, psychosocial, emotional, and environmental needs no less than every 15 days.
• Document problems, goals, interventions and patient/family responses.
