Job summary

Elite Hospice, part of the LHC Group and within the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after normal hours. Join a team focused on compassionate care and patient-family connections.

Responsibilities include triage calls, afterhours visits, eligibility assessments within 48 hours, and thorough documentation to support quality outcomes.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit.
• Respond to crisis calls from patients and families after hours.
• Conduct initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Perform afterhours visits including admissions, death, recertification, and routine visits.
• Identify and reassess patient/family needs at least every 15 days.
• Document problems, goals, interventions and patient/family responses to care.
• Function in any home setting regardless of age, race, creed or disability.
