Job summary

Elite Hospice, part of the LHC Group and a member of the Optum family, seeks an RN to provide per diem after-hours hospice care. The role focuses on comprehensive and timely hospice delivery for patients and families after regular hours, including nights and weekends.

Key tasks include telephonic triage, crisis response, initial eligibility assessments within 48 hours, afterhours visits, and ongoing documentation to support patient and family needs.

Qualifications

• Current unrestricted RN licensure in the state of practice is required.
• Current CPR Certification.
• Minimum one year of clinical experience.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.

Responsibilities

• Triage patient needs over the phone until an RN can visit.
• Respond promptly to afterhours crisis calls from patients and families.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours visits including admission, death, recertification, and routine visits.
