Job summary

Star Hospice, a part of LHC Group within UnitedHealth Group, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends.

Required: active RN license, valid driver's license and reliable transport. Preferred: CPR certification and 1+ year clinical experience. Compensation ranges from $78,288 to $117,432 yearly with comprehensive benefits.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit are available.
• Respond to crisis calls from patients and families afterhours with urgency.
• Conduct initial nursing evaluation within 48 hours of referral.
• Perform afterhours visits including admissions, death, recertification, and routine visits.
• Identify and reassess patient/family needs every 15 days or as needed.
• Document problems, goals, interventions, and responses to hospice care.
