Job summary

21st Century Home Health Services in Northern California seeks a Part-time Weekend Admission Registered Nurse to join our field-based team serving Marin County. You’ll perform Start of Care assessments, admit new patients, and coordinate with the care team to ensure seamless home health services.

Ideal candidates hold a CA RN license, BLS/CPR, a valid CA driver’s license, and know OASIS/SOC processes. We offer flexible weekends, supportive resources, and opportunities for advancement.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Previous experience with IV Medication infusions highly preferred.
• Proficiency initiating IV therapy.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow ups, evaluation visits as needed in designated territory.
• Make home visits to patients in designated territory.
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures.
• Monitor clients’ conditions, reporting changes to Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and Administer IV medication infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
