Job summary

21st Century Home Health Services Inc. is looking for a Part-time Registered Nurse Case Manager to conduct assessments and provide high-quality care to patients in their homes in Walnut Creek. This role offers flexible scheduling and competitive compensation.

Part time Registered Nurse Case Manager

All opportunities at 21HHS require being in the field visiting patients in their homes.

Anticipated Territory: Walnut Creek Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

To be a success:

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
