Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to lead and coordinate hospice care in home and community settings. You will assess patients and families, develop and implement care plans, deliver nursing treatments, and educate families on self-care and disease management.

This role emphasizes collaboration with an interdisciplinary team, on-call coverage, and adherence to hospice standards to ensure comfort and quality of life for patients and families.

Qualifications

• Graduate of an accredited nursing program and RN license in good standing.
• Minimum 1 year nursing experience within last 3 years; BSN with 2 years in health/home care preferred.
• No management experience required; may supervise hospice aides.
• Contingent upon satisfactory references and background checks.
• Excellent observation, communication and problem-solving skills; basic math.

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient/family input.
• Administer medications and treatments per physician's plan.
• Counsel patient and family; provide health instructions and self-care tips.
• Coordinate patient care as Case Manager when assigned.
• Participate in on-call duties and ensure equipment/services are available.
