Job summary

Affinity Hospice Management LLC is seeking a compassionate Registered Nurse to lead home-based hospice care. You will assess patients and families, plan and implement care, and coordinate with physicians and the interdisciplinary team to deliver comprehensive support at home.

The role emphasizes keeping families informed, managing medications, and ensuring safe, quality care. On-call duties and driving to patient homes are required as part of this position.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.
• One year of professional nursing experience in last three years or BSN with two years' experience preferred.
• Must have CPR certification and valid driver's license.
• Ability to lift and assist patients; reliable vehicle required.

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using all elements of the nursing process.
• Develops and revises the plan of care as patient status changes.
• Develops care plans with goals based on nursing diagnosis and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing needs and health instructions.
• Participates in on-call duties and coordinates equipment and services.
• Maintains accurate clinical notes and communicates with physicians and team.
