Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes, with a 3 days scheduled, paid for 4, day shift.

You will review referrals, conduct comprehensive nursing assessments, develop care plans with the client and interdisciplinary team, and educate families to support symptom management and end-of-life care.

Direct LPNs and Home Care Aides, coordinate services with physicians, and document care using the Agency’s standards.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience or one year nursing experience in a specialty.
• Licensure: Connecticut license to practice nursing.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs a complete nursing assessment based on referral data, interview, and observation.
• Develops a plan of care with client and disciplines based on assessment data and goals.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
• Coordinates care with physicians and other providers and documents care appropriately.
