Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in home and community settings, performing assessments and coordinating care with the interdisciplinary team.

Minimum: Associate degree in nursing, CT RN license, and 1 year of nursing experience; BSN preferred, with home care or medical/surgical experience encouraged. Day shift on weekends, with pay for four days.

Qualifications

• Education: Associate's degree in nursing (RN).
• Licensure: Connecticut RN license in good standing.
• Experience: At least 1 year of nursing experience.
• Preferred: BSN and home care or medical/surgical experience.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessments and plans care.
• Coordinates care with interdisciplinary team and documents care plans.
• Educates patients and families on signs, symptoms, and bereavement support.
• Directs home health aides and coordinates resources.
