Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes and community settings. You will review referrals, perform assessments, and develop care plans in collaboration with the patient, family, and interdisciplinary team.

This position emphasizes end-of-life comfort, symptom management, and timely documentation, with supervision of LPNs and Home Care Aides.

Qualifications

• Graduate of an Associate’s or diploma program in nursing.
• One year nursing experience OR one year in a specialty.
• License to practice in the State of Connecticut.
• BSN preferred.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts nursing assessments based on referral data and interviews.
• Demonstrates expertise in symptom management and end-of-life care.
• Develops a care plan in collaboration with client and team.
• Assures bereavement follow-up and documentation.
• Directs LPNs and Home Care Aides in care delivery.
• Coordinates care with physicians and other providers.
• Educates client and family on signs of illness and caregiving.
