Job summary

Hartford HealthCare at Home seeks a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes or community settings. You will assess, plan, and coordinate care to ensure high-quality, cost-effective outcomes. Expect 3 work days with pay for 4, day shifts, and collaboration with an interdisciplinary team.

Minimum: Associate’s or Diploma in Nursing, CT license, and 1 year of nursing experience (or 1 year in a specialty). BSN preferred; home care or hospice exp is a plus.

Qualifications

• Education: Graduate of an Associate’s diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: NLN-approved BSN program.
• Preferred: Medical/surgery or home care experience.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs a complete nursing assessment based on referral data and observations.
• Develops a care plan in collaboration with client and disciplines; documents needs and goals.
• Monitors payer information to maximize care planning and utilizes hospice levels of care.
• Follows interdisciplinary team process to approve non-formulary meds, treatments, and DME.
• Maintains client records using the agency documentation standards.
• Visits clients to assess disease progress and provide education to promote health and dignity.
• Teaches patients and caregivers; prepares family regarding signs and symptoms of death as appropriate.
• Coordinates care with physicians and other providers; refers to community resources.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
