Job summary

Hartford HealthCare at Home seeks a Weekend Hospice RN to provide skilled nursing care in patients\' homes and community settings. You will review referrals, perform comprehensive assessments, and collaborate with an interdisciplinary team to develop and adjust the plan of care.

Qualifications

• Graduate of an Associate’s degree or diploma program in nursing.
• CT licensure to practice nursing.
• One year nursing experience OR one year nursing experience in a specialty.

Responsibilities

• Provides skilled nursing care to hospice clients in homes or community health settings.
• Reviews referrals and initiates contact with client and family.
• Performs a complete nursing assessment and documents care needs and goals.
• Develops and modifies the care plan in collaboration with the client and interdisciplinary team.
• Directs LPNs and Home Care Aides to implement the plan of care.
• Educates patients and families on signs, symptoms, and end-of-life care.

Education

Associate’s degree or diploma in nursing
