Job summary

Hartford HealthCare at Home in Connecticut is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes.

You will perform comprehensive assessments, develop plans of care with the interdisciplinary team, and educate families. The schedule is 3 days with pay for 4 days on a day shift. Connecticut license is required; experience in hospice or home care is preferred.

Qualifications

• Graduate of an ADN in nursing (min qualification).
• One year nursing experience or one year in a specialty.
• Connecticut nursing license.
• Preferred BSN and home care experience.

Responsibilities

• Reviews referrals and initiates contact with clients and families.
• Performs comprehensive nursing assessments and manages end-of-life symptoms.
• Plans and coordinates care with the interdisciplinary team and documents care.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
• Provides patient/family teaching and bereavement support guidance.
• Collaborates with physicians and community resources as needed.
