Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice Registered Nurse to provide skilled nursing care in patients' homes and community settings in Connecticut. You will assess, plan, implement end-of-life care, coordinate with the interdisciplinary team, and educate families to support comfort and dignity.

Qualifications include an Associate's degree in nursing and CT licensure; BSN preferred. One year of nursing experience or hospice/home care experience is desired. Weekend shift.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: Education: Graduate of NLN approved BSN program
• Experience: Medical/surgery or home care experience.

Responsibilities

• Reviews referrals, and initiates contact with client and family.
• Conducts a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status.
• Develops a plan of care in collaboration with client and all disciplines based on assessment data; plans discharge and resources; facilitates bereavement follow up.
• Follows payer information to maximize care planning; utilizes Hospice Levels of care appropriately.
• Keeps client records and documents care using Agency standards.
• Makes follow-up visits to assess disease progress and response to treatment; provides teaching to client/family.
• Teaches patients and reinforces caregiver education; prepares family on signs and symptoms of death as appropriate.
• Refer, collaborate, and coordinate all client care services; consult with physician as needed.
• Directs LPNs and Home Care Aides in carrying out the plan of care.

Education
