Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes or community settings. You will review referrals, perform comprehensive assessments, and collaborate with the interdisciplinary team to create and modify care plans that support comfort and end-of-life goals.

The role requires CT nursing licensure and at least one year of nursing experience, with preferred BSN and home care exposure.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts a complete nursing assessment based on referral data and observations.
• Develops a plan of care with client and team; documents needs, goals, and resources.
• Follows hospice levels of care and payer information for planning.
