Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice Registered Nurse to provide skilled nursing care to hospice clients in their homes and community settings. You will review referrals, perform comprehensive assessments, and develop care plans in collaboration with the interdisciplinary team.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: Education: NLN approved BSN program.
• Preferred: Medical/surgery or home care experience.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts a complete nursing assessment based on referral data and clinical observation.
• Develops a plan of care with client and disciplines; documents needs, goals, and resource use.
• Demonstrates understanding of payer information to maximize care planning and uses Hospice levels of care.
• Directs LPNs and Home Care Aides in carrying out the established plan of care.
• Teaches patients and reinforces client/caregiver teaching and education.
• Refers, collaborates, and coordinates client care services; consults with physician.
• Coordinates care with providers and utilizes IDT for resources.
