Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes and community settings. The role involves assessments, care planning, and coordinating with the interdisciplinary team to ensure high-quality, cost-effective care.

The ideal candidate will have at least one year of nursing experience (or in a specialty), an active nursing license in Connecticut, and, preferably, a NLN-approved BSN.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: NLN-approved BSN program.

Responsibilities

• Reviews referrals, and initiates contact with client and family.
• Conducts a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status.
• Develops a plan of care in collaboration with client and all disciplines based on assessment data and documentation.
• Facilitates peaceful death and ensures bereavement follow up through timely documentation.
• Directs LPNs and Home Care Aides in carrying out the established plan of care.
• Refers clients/family to other community resources as needed.
