Job summary

Carle Health Home Services seeks an experienced Home Care RN/Field Case Manager to coordinate and provide direct and indirect patient care across central IL. You will work with an interdisciplinary team including LPNs, HHAs, PTs, MSWs, and SLPs, and operate with autonomy in patients’ homes.

Responsibilities include skilled nursing assessments, wound care, PICC line care, catheter management, MD communications, and OASIS for admissions and recertifications.

Qualifications

• IL RN license and BLS within 30 days of hire.
• College diploma in Nursing; 1 year nursing experience preferred.
• Valid driver's license and auto insurance proof.

Responsibilities

• Coordinate and provide direct/indirect patient care using the nursing process.
• Perform skilled nursing assessments and wound care (incl. wound vac, ostomy).
• Manage PICC lines/Ports/Blood draws; monitor and educate on health conditions.
• Supervise Home Health Aide and coordinate care across disciplines.
• Complete admission docs, OASIS/485 changes within policy timelines.
