Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to coordinate and provide skilled nursing care in the home setting, working with an interdisciplinary team. The role includes wound care, IV management, PICC line care, and coordinating services across counties.

The position offers autonomy, 24/7 clinical backup, flexible scheduling, and mileage reimbursement. Experience in home health and regulatory knowledge, plus OASIS/485 proficiency, are valued.

Qualifications

• Licensed RN with Illinois registration and BLS within 30 days.
• 1 year nursing experience required.
• Valid driver’s license and auto insurance.

Responsibilities

• Coordinate and provide direct/indirect patient care per plan.
• Perform skilled nursing assessments and wound care.
• Collaborate with the interdisciplinary team to manage diagnoses.
