Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to provide directed and collaborative skilled nursing services across a defined geographic area. You will work with an interdisciplinary team to assess, plan, implement, and evaluate patient care, including complex wound care and line care, while ensuring timely documentation and coordination with physicians and care coordinators.

Qualifications

• IL RN license and BLS certification required.
• College diploma in nursing; 1+ year nursing experience preferred.
• Valid driver’s license and auto liability insurance required.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform skilled nursing assessments and wound care (including wound vac, PICC/ports, drains).
• Collaborate with interdisciplinary team to plan and monitor patient care.
• Complete admission, recertification and discharge documentation within policy.
• Manage case conferencing and communication with physicians, care coordinators and insurers.
