Job summary

Carle Health is seeking an experienced Home Care Registered Nurse/Field Case Manager to join our interdisciplinary team for in-home patient care across designated counties. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments and care delivery in collaboration with LPNs, HHAs, PTs, and other professionals.

Qualifications

• Licensed RN in the state of practice and BLS certification.
• Minimum 1 year of nursing experience.
• Licensure and driving requirements as stated by state policy.

• Perform OASIS/485 assessments and updated documentation.
• Coordinate and provide skilled home care per plan of care.
• Supervise Home Health Aide and coordinate with the care team.
