Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate patient care across counties with an interdisciplinary team, including LPNs, HHAs and therapists. You will perform skilled nursing assessments, educate families, and manage care plans in patients’ homes.

The role emphasizes wound care, IV infusions, central line management, and documentation; strong collaboration with physicians and care coordinators is required. Sign-on bonus available, with competitive compensation.

Qualifications

• IL RN license required; BLS certification within 30 days.
• Driver's license and proof of auto insurance.
• Education: College Diploma in Nursing; 1 year nursing experience preferred.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Complete admission documentation and OASIS/485 changes within 24 hours.
• Perform complex wound care including wound vacs and dressing changes; monitor catheters and PICC/Hickman lines.
• Monitor central lines (PICC/Hickman) and perform dressing changes per protocol; access mediports for IV infusions and blood draws.
• Coordinate care with MD office, care coordinator and other disciplines; document visits and communications.
• Supervise Home Health Aide visits and support case management activities.
