Job summary
Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to coordinate and deliver skilled nursing services across multiple counties. The role involves independent work within an interdisciplinary team, with a $7,500 sign-on bonus for external candidates and opportunities for flexible scheduling and autonomy.
Responsibilities include comprehensive assessments, wound care, catheter and IV management, case management, and ensuring regulatory compliance.
Qualifications
- IL RN license required
- BLS certification within 30 days of hire
- Valid driver’s license and proof of auto insurance
- Education: College Diploma in Nursing
- Minimum 1 year nursing experience in home care or similar setting
Responsibilities
- Coordinate and provide direct and indirect patient care using the nursing process to meet physical, psychosocial, environmental, and spiritual needs.
- Perform skilled nursing assessments and care planning for home care patients.
- Complete admission, ongoing and discharge documentation per policy and regulatory requirements.
- Execute complex wound care, care for PICC lines, drains, and catheters as per agency protocol.
- Collaborate with interdisciplinary team to ensure continuity of care and case management.
