Qualifications

• IL RN license with active BLS certification.
• Valid driver’s license and auto insurance proof.
• 1+ year nursing experience in home health or similar setting.

Responsibilities

• Coordinate and provide direct and indirect patient care per plan of care.
• Perform OASIS assessments and recertifications.
• Manage wound care including wound vacs and dressings.
• Supervise Home Health Aide; coordinate care with interdisciplinary team.
