UnitedHealth Group is seeking a Per Diem Physical Therapist to deliver skilled PT services in patients’ homes, promoting mobility, restoring function, and enhancing safety and independence. You will work with patients, caregivers and the care team to support recovery and reduce hospitalizations.

Responsibilities include improving movement, performing 30-day reexaminations, adjusting interventions, and discharge planning, with coverage that may include Saturdays and PTO.

Qualifications

• Graduate from a CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the state you’re assigned to work in.
• Current CPR certification.
• 1+ year of related physical therapy experience in home health care.
• If less than one year of experience, regional clinical leaders’ approval and mentoring/residency participation may be required.

Responsabilités

• Help patients maintain, restore and improve movement and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as needed, and perform discharge planning.
• Collaborate with patients, caregivers, and an interdisciplinary care team to support recovery and prevent hospitalizations.
