Job summary

UnitedHealth Group is seeking a per diem Physical Therapist to deliver skilled PT services in patients' homes, promoting mobility, restoring function, and enhancing safety and independence. You will collaborate with patients, caregivers, and a care team to support recovery and prevent hospitalizations.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of related physical therapy experience in home health or equivalent.
• CPR certification.
• Driver's license and reliable transportation.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct periodic reexaminations (at least every 30 days), adjust interventions as needed, and perform discharge planning.
• Other duties as assigned.
