Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist to provide skilled in-home therapy, helping patients improve mobility, strength and safety. You will collaborate with caregivers and a multidisciplinary team to support recovery and prevent hospitalizations in the home setting.

Responsibilities include periodic reexaminations, discharge planning, and adapting interventions to each patient’s needs while maintaining quality care and safety standards.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited PT program.
• Minimum 1 year of PT experience in home health or equivalent.
• If less than one year, approval from regional clinical leaders and mentoring or residency required.
• Current CPR certification.
• Driver's license and access to reliable transportation.

Responsibilities

• Provide skilled PT services in patients' homes to promote mobility and independence.
• Reexamine patients every 30 days, adjust interventions as needed and perform discharge planning.
• Collaborate with patients, caregivers and interdisciplinary care team.
