Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist to deliver skilled PT services in patients' homes, promoting mobility, safety and independence. You will collaborate with patients, caregivers and an interdisciplinary team to support recovery and reduce hospitalizations.

Responsibilities include reexaminations every 30 days, plan adjustments, discharge planning, and performing other duties as assigned. Requires CAPTE‑accredited program graduation and current state license.

Qualifications

• Graduate from an CAPTE‑accredited physical therapy program.
• Current license to practice physical therapy in the assigned state.
• 1+ year of home health care PT experience or approved mentoring/ residency.
• Current CPR certification.

Responsibilities

• Provide skilled PT services to patients in their homes to promote mobility and independence.
• Perform reexaminations every 30 days and adjust treatment plans as needed.
• Discharge planning and collaboration with care team to prevent hospitalizations.
